As a patient of WWFP who may require hospitalization, you will be seen by a WWFP Hospitalist. A Hospitalist is a Physician who provides general medical care to patients in the hospital by leading the hospital medical team and coordinating care for inpatients.
Dr. Kovach, Dr. May, Dr. Ullom, and Dr. Yang provide Hospitalist services for WWFP Patients at Ascension Via Christi St. Francis and newborn care at Wesley Medical Center and Ascension Via Christi St. Joseph.
The WWFP Hospitalist Team sees patients 24/7/365.

Upon discharge from the hospital, WWFP Hospitalists will contact WWFP Transitional Care Management (TCM) Team to coordinate your care. The TCM Team will contact you via phone to reconcile your medications and ensure your understanding of any medication changes.
The TCM Team will schedule an appointment with your PCP who will review your recent hospitalization. This continuity of care provides a smooth transition for patients that improves
care and helps prevent unnecessary readmission, thereby reducing costs.
